Criteria for Admission - Teen Mental Health PHP

Applicants seeking admission to the teen mental health PHP must be 12 – 17 years old, and have a primary mental health diagnosis including:

  • Depressive Disorders
  • Bipolar and Related Disorders
  • Anxiety Disorders
  • Trauma and Stressor-Related Disorders
  • Attention-Deficit/Hyperactivity Disorder
  • Substance Use Disorders with a Primary Psychiatric Diagnosis

Admissions Process:

  • Call the Admissions Department at 1-844-HOPEWAY or submit an inquiry form via the website.
  • Our Admission Specialists will provide general information about our program and conduct a clinical assessment with the adolescent as well as an interview with the potential client’s parents or guardians. This will help us determine if the program can effectively meet the individual’s needs. If psychological or psychoeducational testing has previously been completed, those records will need to be reviewed by your Admissions Specialist. 
  • We will provide you with the Insurance Verification Form to complete that allows us to check insurance benefits and link you to one of our Financial Specialists who can answer any questions you may have regarding the financial aspects of service.
  • Once you complete the Insurance Verification Form and copy both sides of your insurance card, you can submit the documents to your Admissions Specialist. 
  • Lastly, our psychiatrist will complete a psychiatric consultation and the adolescent will admit the following program day.

Insurance Information:

Insurance Verification Form

We are in the process of becoming an in-network provider for our teen mental health and teen and young adult eating disorder programs. However, we are currently out-of-network during this process. To ensure the financially responsible party has a full understanding of the current financial process, we will verify out-of-network benefits and connect the financially responsible party with a HopeWay Financial Specialist to answer any questions. As an out-of-network provider, we will attempt to receive authorization for services and file claims for our clients. We will then collect an upfront payment for services and the client’s insurance company will reimburse the subscriber directly based on their specific insurance policy terms. Through the generous support of our community & donors, we do have financial assistance available for those with a demonstrated financial need and will work to meet each families needs.

Exclusionary Criteria

Applicants seeking admission to HopeWay's Teen Mental Health PHP will not be admitted if the following are present:

  • The need for a higher level of care or acute hospitalization (which may include acute suicidal, homicidal or self-injurious behaviors or active psychosis or mania);
  • The individual has a medical condition that cannot be adequately treated in our setting; 
  • The individual has a primary diagnosis of conduct disorder, oppositional defiant disorder, or the individual has displayed ongoing violent behavior towards oneself, others, or property;
  • The individual has a diagnosis of Intellectual Developmental Disorder;
  • The individual has a primary diagnosis of Autism Spectrum Disorder, Substance Use Disorder, or Eating Disorder;
  • The individual has significant resistance or unwillingness to engage in the program.
